Hospitals, in an effort to solidify referral patterns, enhance primary care and grow their business, have begun to offer their Epic Community Connect technology platform to the community. Mr. Jacobs is responsible for leading HCI’s Technology Outreach Service (HCI Reach). He brings extensive experience in program and project management from healthcare organizations across the country, leading and coaching IT leaders in the nuance of community project leadership and advising organizations on how to achieve success in the community. In support of hundreds of community EHR Go-Lives, he has led or advised on programs in Michigan, Florida, Pennsylvania and California for multiple vendor products, including Allscripts, McKesson, Cerner, Epic, NextGen and eCW.
